How do doctors specialize in more than one thing?

This may sound stupid but, i love to watch the show HOUSE and ive noticed that house specializes in different categories. ex. neurology, infectious diseases.would it take extra years in med school or would you take two cles a day? im sorry if you dont understand me but please try your best to answer. thank you!
